I've gone through the general fixes, and I believe I've ruled them out with a this exception:
#6 - The default Joomla template allows me to log in properly.
To the best of my ability, here is the additional information:
1: PHP version 5.x
2. CB version 1.3.1
3. Joomla version 1.5.22
4. As stated in original post, when logging in through the CB Login module, I simply get another login page as shown in the earlier attachment. None of the "reminder" links result in an email being sent to the correct email address. I also cannot successfully create a new registered user.

I did notice that I intentionally set the CB Login module name to "CB Login", but when it takes me to the next login page the title simply says "Login".
