You didn't supply a name. All your fields except 1, which only has 1 character (a digit) has a value. You also set the Name validation to "At least 6 chars, 1 a-z, 1 A-Z, 1 0-9, 1 special" instead of the default "Any string ( /.*/ )". This means you need to supply a name like aZ0#aa for it to be valid. I suggest changing it back to "Any string ( /.*/ )".
