Are you using a custom backend template? Ensure you're using default backend template. Aside from that I don't know what to tell you. Either the HTML is missing where it'd add the button or there's a fatal stopping the JS from loading or being added. Ensure you're testing with debug mode and maximum error reporting enabled in Joomla global configuration. If issue persists after checking all this then PM backend super administrator login credentials and can try taking a look.
