You can disable avatar upload approval within CB > Configuration > Moderation and set "Require Upload Image Approval" to "No". The ProfileBook Wall approval is user by user and not moderator based so you can't really force approval and needs to be enabled/disabled on a user by user basis using the cb_pb_autopublish_wall field.
