Are you able to reliably reproduce it? If not then it's probably caching. If you're Joomlas page caching enabled then disable it. It doesn't work in dynamic environments. If you can reliably reproduce the issue it's probably something installed or configured to act on logins. Something like CB Auto Actions acting on login could cause issues if you've configured an auto action incorrectly or could be a Joomla user or authentication plugin within Extensions > Plugins.
