Quizz Add On

Hello, I'm looking for a add on/extension that let's me put a question on the website, only visible to certain user groups.

It also has to have a form below the question where they can put their answer.
And It must be so, that they can only answer the question 1 time per account, and only if they have an account.

The add on has to send an email to the admin's mail with The username of the person who answered, to which user group he or she belongs, and his or her answer.

Also the add on must be able to handle multiple forms, based on some factors.

for example:

Question; What's the color of a banana?
Only visible and answerable for usergroups: Public & Registered.

Question: What's the name of our planet?
Only visible and answerable for usergroups: Super user.


Is there any way to do this, with intergration of CB?
OR can someone make an add on like this?

Sounds like you're looking for a survey processing extension .

And It must be so, that they can only answer the question 1 time per account, and only if they have an account.

...which would likely be built in to a survey script but I haven't checked.
