NEED HELP pre-formatted blogs; please!

Okay, here is the deal; I'm using CB and have Mamblog installed. What I'm wondering is if there is anyway for this to happen;

I want my users to be able to click on "NEW BLOG"
then it takes them to a list of selectable topics (we'll use actors). So let's say the list looks like;
Brad Pitt
Paul Newman
Nicole Kidman
Keira Knightley
Natalie Portman
Once they select an actor I want it to take them to a "blog" with pre-formatted fields. So the blog might look like:
Brad Pitt
Best Film (blank field)
Best film of the last 5 years (drop menu of films from the last five years)
Better with (check box of Jolie or Aniston)
Should have won an ocsar for what role (blank field)
Additional (large blank field)

And I'd like to have it different with each "actor" or topic.
I know this is a big question. If anyone out there has the time and knowledge to write out how I go about this I would be greatly appreciative. Seriously, I'm stressing so much about this one because I can't figure it out. Please let me know! Thanks so much!
