fields in registration form

Hello,

I'm new in Community Builder and unfortunately I don't know how to prepare registration form which I really want. Let's say that I want to create registration form with fields: "First Name" and "Last Name". Bellow these fields I would like to have question like: "Do you want other logged in members to see your last name?" which can have two values YES or NO. But how to really connect this question to "Last Name". I want to allow user to decide during registration whether his last name will be visible for others or not? Maybe some CB plugin can do this?

CB Configuration area allows you to setup the firstname lastname format.

For other issue you need the CB Privacy plugin (available for Advanced and Professional members)
