CB has one single registration form.
You can however adjust which fields (in addition to the mandatory username, name, email, password) appear on this form with 2 methods.
You can use CB Conditionals plugin to conditionally show/hide a fields based on another field value. This plugin is part of the Professional membership package.
You can adjust registration form also with CBSubs based on the selected subscription plan.
