I a way of creating custom forms that can be linked to a user?
I am creating a demo site for a doctors surgery, and one of the request that was made was a repeat prescription request form.
The idea is the gp write the medication list on a form for a user (patient),
when the user requires more medication the access their profile moves to the medication tab which shows the form ticks the relevant boxes then submits the request to their Gp
