This can be done without hacks using an incubator project. Please see my previous reply. It takes a bit of configuration, but takes zero lines of code. It also requires gallery to be enabled.

Ok if I've understood, I go in configuration > image > image gallery > set to Yes.
Then I open CB fields plugin and I create a new field:
triggers onAfterUserRegistration
user automatic
access All registered users
conditional Gender select operator (?)
field Profile image
operator Set Field = Value
Value (?)
How can I complete this configuration? Can you suggest it, please ?
The below should work fine assuming your feilds values are "Male" and "Female".
Fields 1
Trigger: onAfterUserRegistration
User: Automatic
Access: All registered users
Conditional:
-Field: Gender
-Operator: Equal To
-Value: Male
Field: Profile image
-Operator: Set ( Field = Value )
-Value: gallery/male.png
Fields 2
Trigger: onAfterUserRegistration
User: Automatic
Access: All registered users
Conditional:
-Field: Gender
-Operator: Equal To
-Value: Female
Field: Profile image
-Operator: Set ( Field = Value )
-Value: gallery/female.png
You need to ensure male.png and female.png is in the below folder.
