Well normally in such scenarios there is at least an email address to link the user with the external item. In your case you've zero relation between survey and user, which is a huge problem as you can't be anonymous and not anonymous at the same time; there needs to be a relation or it won't work.
Cookies aren't reliable as it takes me 2 seconds to fake a cookie on any website; the only reliable cookies are session cookies which just store a unique session id generated by PHP and that helps the viewing user relate to a logged in user by matching against a session id in database, which could work for you I suppose to keep it secured.
