Lost Username / Password Emails not working

I've seen a few posts on this but no previously suggested solution has worked for me yet.

CB 1.9, Joomla 2.5.9

After selecting Lost Username or Lost Password and filling out form, login form is redisplayed without an error message.
No reminder/reset email is received.
User password reset count is not incremented in the user manager.

Other emails from the system are sending / receiving fine.
Notifications are set to use admin address.
Admin email address has been entered into Registration Email Address and Registration Email Reply To Address
Have reviewed global site email config, but don't see any problems there, have tried with SMTP and PHP emailer.

Any help appreciated. Would also like to know if anyone has an admin tool for bulk resetting passwords and sending notifications.

Admins, for security purposes, can not reset their passwords. Ensure you're using a normal registered user when testing. Please provide what browser you're using; if possible try a different browser. Enable debug mode and maximum error reporting in Joomla global configuration during testing to expose errors.

Thanks. I've been testing with a non-admin user in Safari and IE 9. I'll turn on the error reporting to see what that uncovers.
