No idea, it seams to be working perfectly fine in all my tests. You can only update 1 field at a time. If you click to Ajax 1 field, it'll close the others. What about the image field specifically isn't working? When selecting to upload from the dropdown it's not showing the file field?

On a new fresh user account that had never upgraded any image, when I click over the image to upload a new one, nothing happens. Both 2 images (avatar default and other image).
Then, I go to profile view edit and I upload and image in the first avatar field. Now I can click and edit this image in profile view, while the second image still not work with ajax in profile view. So I must to upload first and image in edit view. Then it seems to work fine.
Am not able to confirm. Please PM backend super administrator login credentials as well as frontend login credentials with exact steps to duplicate so may take a look.

Have reviewed your install and it's due to setting "Show empty fields" to "No" and not giving the AJAX usage a placeholder. I've set the placeholder for your two image fields to "-" and it works fine now. Note no one will see the placeholder, but will allow it to ignore the hiding of empty fields usage. Please also confirm if is working fine for you now.
