Well the first goal was to get rid of your errors. So it's good that they're gone now.

Try CB > User Management > Mass Mailer (click your user first so it only sends to you) to see if your site is even sending emails. You may have a configuration issue with your mailer settings within Joomla global configuration.

This is odd. When I click on the Forgot Login? button, I initially get the attached screen. However, once the page finishes loading, the only thing showing is this:

Lost your Username or your Password ?
Reminder needed for:
Lost Username
Lost Password

When I debug, I no longer get any errors. I'm at a loss and frustrated.

It's displaying perfectly fine. JS does not execute until the page completely loads. You need to check the checkbox of what you want to reset.

Please see my above reply and test to make sure emails are even working on your site. Please also understand admins can not use forgot login for security reasons and you need to test with a regular registered user.
