CB Connections can be used to connection the two users together. It's like Friends on other social sites. Aside from that I suppose you could add a field only moderators can edit that you supply the spouses user_id to. I suppose you could also pair them by lastname and use CB Query Field to query the database for all users with same lastname and display them as a field on their profile.
