I suggest using the onAfterUserApproval trigger. I can't see any reason why it'd do a full replacement unless the user didn't properly exist in CB and gids array isn't populated. Will investigate the action further and see if how it pulls what groups a user already have needs to be adjusted.

There's likely an issue with it grabbing the existing usergroups. I'll know more once I investigate for next release after CB 2.0 goes stable (all incubator projects will see a new release after CB 2.0).
