Where to edit offline payment function in CB Subs was created by rustle44

Hi Guys
I am at a total loss as to where the html output is for the 'pay offline' function

I was able to style the invoice & options in the back end

but what I am looking to edit is the html output of this page

The payment slip can be styled by editing the Offline payment gateway within CBSubs > Gateways. The invoice can be modified within CBSubs > Settings > Display > Invoices.
