Joomla doesn't use Itemid from External Joomla menu links. You don't need to create a Joomla menu link. You need to edit the Pending email and replace [CONFIRM] with your own URL as exampled above.

Hi Kyle,

now i found out what you meant by "You need to edit the Pending email..":
i have to edit some stuff in the CB configuration (Duuh, that was hard to find out ...)

OK i edit the "Pending email body" => [CONFIRM] to
[http://www mydomain com/.../index.php?option=com_comprofiler&task=confirm&confirmcode=regb5d0db0d91d94dad6004d3d657aef5180047fddf&Itemid=147]

Now this works, the user is confirmed and the right template is displayed.

Thanks again

PS I i only would replace [CONFIRM] with my domain [http://www mydomain com] confirmation would not work...
