I figured out what it is. Your code to set a field "required" in "conditions others" is buggy. As soon as I took my Query Drop Down (Single Select) field out of that condition it works as you claim it would (with one empty option at the start).
I guess this needs to be fixed in 1.9.1 even though 2.0 is just around the corner.
CB Conditional shouldn't remove options. It toggles the required CSS class and the show/hide of the required icon. There could be an if check bug in it causing it to affect display, but it's not an issue in CB but an issue in CB Conditional. Have created a bug ticket to review the required state conditional for next release of CB Conditional after CB 2.0 goes Stable.

It's likely a bug in CB Conditional (have not tested and confirmed). It does not have anything to do with CB 1.9.1 core. CB Conditional will receive a new release after CB 2.0 goes Stable. Same applies for all incubator projects.
