I recommend contacting the developer of CB Public Mail to upgrade their plugin then. We love that developers can and do improve on core functionality. They're welcome to upgrade their plugin and add whatever functionality they feel necessary. It's worth noting the email form display has a trigger (onAfterEmailUserForm) to add new input rows of which can be processed on the onBeforeEmailUser trigger so it is possible for a plugin to extend the email form.
For core CB complicating the email form just does not make sense for the majority as there is no UI to add fields to it. Once we're able to generate XML driven frontend layouts and have a layout editor it maybe possible to easily have additional fields, but for now I don't see this changing.
