I ask because there is a Joomla extension that allows multiple uses with the same email. I'd like to know whether (and how) CBSubs would break if email address were not unique.
No idea. Never tested CBSubs in such a scenario as it is specifically not allowed by Joomla. Going against core design is asking for problems. What those problems are I have no clue. So my best advise is to try it and see what happens. My guess is it'll work perfectly fine as CBSubs doesn't care if the email address is unique or not. The user will likely end up getting multiple emails though if the two users have their own subscriptions.

Seems to work fine. I've poked around the Joomla forums, and I could never figure out why they had this restriction. All I've found is general comments about security with no specifics. And I've never been able to come up with a scenario in which someone could take advantage of this to masquerade as another user, unless he already had that user's password (in which case, why bother?).
If duplicate emails were allowed then spammers could easily abuse it. They'd create 1 email then spam register 100's of accounts. There's a couple of other reasons (e.g. impersonating another user). Generally 1 account per unique email is the most secure way to go about it. You can't see a reason to not allow this, but I can't see a reason to allow this; lol. At any rate if it's working fine for you then you shouldn't really have any issues as I can't think of any checks that ensure email is unique besides at time of registration.
