Create a new CB Menu action with the Triggers set to onPrepareMenus as it's the only trigger that action can function on. Next configure the rest of the menu action as needed to add menu items. Conditionals, access, etc.. can all be used to determine visibility of the menu item as well. Example Menu action configuration as follows.
Type: CB Menu
Triggers: onPrepareMenus
User: Automatic
Access: Everybody
Title: Test
Type: _UE_MENU_EDIT
Class: _UE_MYMENU
Position: Menu Bar
URL: index.php
