I've yet to receive the credentials so I am unable to investigate any further. When possible click the "send me a private message" link in my signature and provide super administrator login credentials. I will then investigate when possible. I will likely need to add debug code to the plugin to see what is being output before the image header to cause it to fail so if this is a very active site you may want to consider cloning it to avoid disruption for your users.
