Captcha Image not displaying

Captcha Image not displaying was created by wither98
I am having issues with the anti spam. The Captcha image that is supposed to display the code isn't showing anything but a torn page icon. Any suggestions for a fix?

Please give exact version numbers for Joomla, CB, CB Antispam.

they are all the most current versions of each. cb anti spam says 2.0.0.CI.2015.0… with a clean install not updated from older versions

It's typically due to the endpoint being unreachable or you've a fatal error causing the endpoint to fail to output the image. Press F12 in Chrome or Firefox then click the Network tab. Now refresh the page and see if anything shows as red and if anything does see if it leads to a pluginclass=cbantispam page to see if the URL is unreachable. If everything looks ok there then enable debug mode and maximum error reporting in Joomla global configuration and refresh the page. Now check server error log for any fatal errors as maybe one of the PHP image functions is failing.
