I believe this maybe due to who the email is being sent from. When using that form it specifies a from address of the user and not of the site. The other emails that are working send from the site and not a user. Within CB > Configuration > General set "Emails Sent From" to "Admin Email Address, with Reply-To User Email Address" then see if your issue persists.
