2. The login at the top of the page is CB and works fine.... however the one (screenshot) in the middle of the page will not work and will display the warning error message.
Please note that if I login using the other login form (CB login module at the top of the page), it logs me successfully.
I would ideally like to NOT have the one in the middle of the page.

Within Extensions > Plugins ensure "System - Community Builder" is public, published, and ordered as the first system plugin to load. Next edit it and under "Parse URLs" set both "Redirect URLs" and "Rewrite URLs" to "Enabled". Normally "Rewrite URLs" isn't needed, but in some cases it needs to be enabled and is disabled by default.
I would ideally like to NOT have the one in the middle of the page.
You won't be able to prevent that really as that's just how Joomla access protection works. It always redirects to login.

It doesn't exist for CB 1.x. You'll need to dig through the archives and download then install CB Core Redirect in that case.
