Unable to get it to work with the bootstrap alternative layout, however if I set alternative to default and modify the default_logout.php file int TEMPLATE/html/mod_cblogin folder it works.
You would have to copy both the bootstrap and bootstrap_logout then select the new bootstrap layout as the module layout. It should then properly switch to the logout layout during logout display. Aside from that am unsure what to advise. It uses the same method as Joomlas login module for selecting the logout layout.

Strange that it works with default and not bootstrap. You could try copying the bootstrap layouts then rename them to default or naming them something other than bootstrap (e.g. mylayout.php and mylayout_logout.php). Sorry, not familiar enough with Joomla layout overrides to suggest any further workarounds.
