What is this I'm seeing on this client's workflows module?

I see this on all CB pages front and backend.

Also, I see this when I try to save Config:

Error

Failed to save Configuration because: Name missing!

Nope, no idea what that's coming from. My best guess is a 3rd party extension or 3rd party CB plugin.

odd, this client was tagged by host for having malware. I have no suspect 3rd party CB plugins but this is only seen on CB pages front and backend. Everything is updated to latest version. This particular client has a very poor host that does not allow virus scanner or mod security in cpanel. I did find another CB user with same issue from last year but there was no further replies on his thread:

Disable non-core Joomla extensions and CB plugins 1 by 1 until it goes away to see if it's something installed causing it. It's probably a 3rd party CB plugin that has an echo in it outside of its plugin class and isn't properly coded as a CB plugin.
