It's making sure you're done, because if you don't click save after changing a title or description then your changes will be lost. This is being improved in a future release where it won't need to ask you anymore. See the below ticket to keep track of this.
In short if there are changes to be saved it changes to Save & Done and does the save. If there isn't any changes to save it just says Done. In both cases confirmation is being removed.
