Hi,

When I go to Fields or Lists on backend, and hit the NEW button, I get the following (attached) error message, and I am then taken to the last page. What could have caused this error and how could we correct it?

You're failing spoof checking. Do you have cookies disabled? Is your browser up to date? Have you tried a different browser?

Hi Kyle,

The cookies are not disabled. The browser is up-to-date, and yes, this error can be reproduced on all browsers and different systems I tried on.

Any idea how this could be troubleshooted to find out the exact cause of the issue?

Disable all caching then purge the cache and see if issue persists as it's typically a caching issue. I've no idea why it'd be happening in backend though as there should be no caching in backend. If using a custom backend template please switch to default Joomla and retry.
