I've found the default template in /components/com_comprofiler/plugin/user/plug_cbqueryfield/templates but it only has display.php with the HTML_queryfieldDisplay class, and no indication of how the results of the query in the Query Field - Query Parameters are passed into the .php template for parsing. I can't find any indication of this in any documentation or forum posts. How is the query returned into the .php? Once I know that, I'll take the rest.

Is it an array? A recordset object? The query will have multiple columns in multiple rows that I want to format specifically in PHP.

The query has already been loaded into the database API. It just needs to be executed and any display behavior you want added to its results. The default template for example simply calls $database->loadResult() to grab its results. If you want an array of data you'd need to change the function call used for different results (e.g. loadResultArray, loadAssoc, loadAssocList, loadObject, loadObjectList, loadRow, loadRowList).
