The only except is don't use the Update On included with CB Query Field. It's deprecated and will be removed in the next major release. Use the Update On provided by CB Core Fields Ajax. Aside from that change the rest of the tutorial is fine.

I am really not sure where its going wrong that is why attaching here screenshots of state and city setup.

Does city not reload its display after changing state? You should see a spinning gear icon to indicate it's reloading after the change. Ensure CB Core Fields Ajax and CB Query Field are both public and published in CB > Plugin Management. Also ensure Profile View Access Level within CB > Configuration > User Profile is set to something the user can access (e.g. Public or Registered) otherwise the ajax endpoint won't be reachable.
