My guess is a fatal JS error is breaking the JS responsible for playing the audio or the ajax endpoint is blocked for some reason. First ensure CB AntiSpam is public and published in CB > Plugin Management. Next ensure the tab the Captcha field is on is published and public. This should guarantee public access to CB AntiSpam endpoints. Next try to use the refresh icon to see if the image refreshes. If it does then the endpoint can be reached fine.
If refresh doesn't work then it's guaranteed to be a fatal JS error breaking it. You'd need to press F12 to open your browsers developer tools then select the Console tab. Next refresh the page and attempt to play the audio to see if any fatal JS errors are present in the Console tab.
