Are you sure the firstname field is displaying and not formatname field? If the name format in CB > Configuration > General is set to display name it will show their firstname. Also note CB Moderators and the users self is exempt from privacy controls and will always see the field. Be sure Moderator View Access Level is also set correctly in CB > Configuration > Moderation as it's possible to accidentally give everyone moderator permission making CB Privacy appear to not work.
