It only logs the activity if the user is confirmed and approved. Is the user confirmed and approved? Check within CB Activity > Activity to be sure the activity wasn't logged as it's possible it's just not visible.

Users are confirmed and approved.. I've got no idea
How are users registering? This only acts on CB triggers and does not act on backend user create. So if you're creating them in backend or they are not registering through CB it won't create the activity.

No, they register on the frontend with cb-register and Joomla register is inactive
Strange, not sure what to suggest. Appears to be working fine in my tests. Please be sure your entire install is up to date. If still not working try using the "Reinstall System Actions" button in CB Auto Actions > System Actions and see if it works then. Only other thing I can think of is if you've another system action acting on onAfterUserRegistration or an earlier trigger before the system action and are interrupting the process (e.g. a redirect).
