Is it possible to create an E-mail auto action that sendes an e-mail to the user after admin has edited the users profile in backend and added a user group with a certain ID ?
Yes, use the onAfterUpdateUser trigger and set Access to the usergroup that was added in an Email action and it should do exactly that. onAfterUpdateUser is only fired during backend user edits so will have no impact on frontend. If you need to also only do this once then you'll probably need to try the below condition.
Field: Custom > Value
Custom Value: [var1_gids]
Operator: Not Equal To
Value: [var2_gids]
