Text "[formatname]" is showing where the actual username should be showing was created by a2zcs

In many places the text "[formatname]" is showing where the username should be showing such as upon login, snipboard.io/nf2mVW.jpg
This is also happening in all notifications that are sent out by CB

Any ideas as to what is causing this?
