You'd have to make a custom template for CB Activity, edit the template file, and then yes it'd display whatever header/footer/etc.. you'd like. To do this navigate to the following directory.
Create a new folder. For the purpose of this example we'll just call it "custom", but you can name it whatever you like (letters and numbers only). So now we should have the following folders in this directory.
You can freely edit "display.php" however you like. It's responsible for that display you see in your article. In a future release these will just be Joomla template layout overrides though so will get easier later.

Ok, thanks. I will give it a try. My workaround was to create the desired header in a custom HTML module and drop the shortcode [cbactivity: comments id="COMMENT_ID" /] just below it. Then I can easily insert the module into the article. Seems the shortcodes will not render in the custom HTML module??
That won't work since the article comments stream needs information from the article to render properly. If you had CB Auto Actions you'd be able to act on the same event CB Activity is to insert its comments stream to insert custom HTML in the same location, but I don't think Joomla has a module position for inserting inside the article.
Is inserting text there exactly necessary or do you just need some further visibility improvements. Like making the comments appear larger? Article comment UX is pretty much the same across all site (at the bottom) so I don't think your users should have a hard time finding this. Making it bigger however should be relatively trivial using some CSS. For example the below will increase the scale of the comments by 25%.

Maybe this would work better for you instead? You can apply custom CSS to CB using the below or using Joomla's user.css template file.
