Registration page does not display registration fields

I haven't been able to find anything similar to this issue posted anywhere...

I installed CB a few weeks ago and had everything working fine.
Now when I log on and try to test the Registration process, the page comes up with the button to Register, but none of the registration fields are showing! (image attached)

I've gone back and checked that everything was installed correctly

In the CB Configuration Manager > Registration
I have Allow User Registration set to 'Yes, independently of global site setting'

In User Manager > Options I have Allow User Registration set to 'No'

I'm using Joomla 2.5.8
CB version 1.9
PHP Version 5.3

I'm hoping that I've just missed something simple, but I'm banging my head against the wall right now! Thanks for any help.

make sure all your tabs and plugins are published public.
