I am using CB latest version.
I have set the moderator to approve the registration. The new user will receive the email: pending registration.
However, the moderator does not receive an email to approve.
In my opinion, everything is set up correctly.

Approval is done after email confirmation. You'll receive an email to approve them once they confirm their email address. If you're not getting such an email after they confirm then edit your user in CB > User Management and ensure "Receive Moderator Emails" is set to "Yes".
