I have tried every possible combination of settings, but the problem persists.
I have 2 main plans, both of which require email confirmation before payment.
One plan does not require moderator approval, the other one does (before payment).
Pertinent global CBSubs settings:

Regardless of how I change these settings, if a user registers and then tries to login in before confirming email, they are shown a page that says that their subscription is expired with a button to Pay Now. If they click the button, they are able to pay, even if they haven't confirmed email or, in the case of the plan requiring moderator approval, even if they have not been approved.
I will send you login credentials if you want to take a look.
Ok, looks like this is a known bug with the expired subscription check happening prematurely. The workaround is to actually set "Allow Free Registered Users (without subscriptions)" to "Yes" to skip this behavior. I've done so on your install and confirmed login is correctly blocked now. We'll be investigating this issue further to fix the check to work properly.

I changed the settings as you suggested and it does prevent newly registered members from logging in prior to confirming their email address. However, when they confirm email and then if they choose OFFLINE PAYMENT, they are now able to login and do all of the things that a member of group Registered is able to do even before the offline payment has been marked as paid...
Users aren't blocked during pending payments. An offline payment when confirmed goes into a pending state. So they'll have regular registered user access during that state. If payment is not confirmed they'll be blocked again. If payment is confirmed they'll have an active subscription.
