Could be a bug, not sure.

Thinking that one of the fields that is required is the Joomla name field (you cannot have a user created without it), so if CB name format is set to first-middle-last, then I guess all become required (even if you have lowered middle name requirement).

I tried an another site with a clean install of CB and found the same problem. If I set middle name to "required" for the registration form, then the admin setting to ignore validation requirements doesn't seem to work. I can't edit a user without entering a middle name (even though the "required" icon isn't there.)

This is starting to become a real problem for one of my clients - they can't edit any users in the back end without being prompted to enter a middle name. Is there a way I can disable "required field" validation checks in the admin area altogether? Maybe some line somewhere I can comment out? That would be very helpful

There's no solution as of yet. As it's a part of the Name field it follows the same validations of it. The first/middle/last fields are just split field of the name field and combines them into Name. You can find our findings and monitor progress with the bug ticket below.
