Register a user then afterwards check the _users database table to see if it stored as plaintext. This will help get an understanding of what the problem is. Also review if you've anything installed that's acting on the registration triggers such as CB Auto Actions and disable them then retest.

Not sure what the problem is. I was able to register and login perfectly fine after clicking the confirmation link. You've numerous users that never clicked the confirmation link (looks like test accounts?) so they of course can't login until they do so.

No idea, your site seams to be throwing a 403 error and can't review any further. There's no reason for login by username only or login by email only to not work. It's the same exact login process as using login by username, email, or CMS.
