Joomla 3.2.2 is also out. It maybe worth a try to upgrade from 3.2.1 as well to see if it could be somehow Joomla related.

Yes, we are planning both a Joomla 3.2.2 upgrade and CB reinstallation over the week-end.

I will know monday if he has fixed it or not...

We upgraded to Joomla 3.2.2 and did a full re-installation of CB 1.9.1.

However, when we add a user via CB, the issue still persists.

See the error messages in the attached screen shots.

Did you alter the auto increment values in your _users or _comprofiler table? Sounds like auto increment is off. Only way to fix that is to navigate to both tables and ensure both the auto increments are correct and the same.

I dont't think we made any changes in the tables voluntarily. But I I am asking our DBA to check this.

I've disabled everything on the cloned site. This did not resolve the issue. I then re-installed CB 1.9.1 over top of it self on the cloned site and issue immediately resolved. So someone on your staff or something installed has modified CBs core code. Please re-install CB 1.9.1 on your live site and see if that resolves it. If not you'll need to shut off 3rd party extensions 1 by 1, re-install, then re-check and repeat as needed.
