I set up an auto action type "Email". I have no conditionals. Access is set to "Registered". No email gets sent when the admin clicks on the Approved icon.
Please provide a screenshot of your auto action configuration so I can review the parameters.

Name the image file something unique with no special characters and no spaces. It should then upload fine. You can also use a service like imgur and link it.
