Set access to Everybody then retry to see if working. If still not working set User to "Manually" then "Variable 1" and retry. Please also describe how you're approving a user as that trigger only fires on user approval from a moderator.

"Set access to Everybody" > that solved it, thanks
but with that comes the next problem. an action that worked before has stoped now. please see the screen shot. what am I missing here? even disabling the other actions did not help
