SOLVED: It is the Tab Permissions that were set to Registered that were causing the problem. Once I changed them to Public, the fields started to display.
I tried changing the User Manager > Guest User Group from Guest to Public. Didn't make a difference and the site that is working has it set to Guest.
I tried changing User Manager > View Access Levels so for Guest both Guest and Public are checkd and for Public both Public and Guest are click. Didn't make a difference and the site that is working only has the one item clicked.
I agree that there is a permission issue somewhere, I just can't figure out where. Any other ideas or what to try or where to look?
