Contact your host and ensure mod_rewrite is configured for your server. How did you create your menu items? Are they standard Internal Joomla menu items? If they're External menu items where you supplied the URL then they need to be stores as non-SEF.

The login module is being broken. Its form element is being removed. I've no idea what is doing that, but it isn't CB doing it. Between this and your other issues you've something causing some serious alterations or conflicts on your install.

I'm out of ideas. I don't know what's wrong with your install. I am unable to duplicate on a clean install created specifically to test the issues you've been experiencing and I can not duplicate them. These issues appear isolate to your install. Likely culprit is 3rd party extension.
I suggest starting a second Joomla install and carefully re-installing extensions 1 by 1. I can take a look if you PM super administrator credentials, but I don't know what good it'll do.
