Yes, this is very difficult to trace. I think I may have resolved it, though. I found an extra/extraneous user profile link I had created and I removed that. We have had no further issues (save for some registrants who didn't recognize the return/confirmation link as a link) since.

The extra menu link might account for what you experienced - hope things are solved now.
